Dear Duval County High School
Seniors,
The Duval County Council of
PTAs is pleased to offer five $500 scholarships to students graduating
from Duval County schools in 2015.
To be eligible for this
scholarship, an applicant must:

Be a 2015
graduating senior of a Florida high school;
·
Be a member of
your school’s PTA/PTSA or the Sunshine State PTSA; Note: the PTA/PTSA must be
in good standing. Verify this information about your school’s PTSA at
http:www.dccpta.org/goodstanding. If your school does not have a PTSA,
you may join the Sunshine State PTA at http://floridapta.org/index.php/about-us/membership/sunshine-state
by completing the form and returning it with payment.

Have an
unweighted GPA of 3.0 or higher
·
Submit completed
application form, including reference letters and transcript in a sealed
envelope, and essay pages to the DCCPTA, Attention Scholarship Committee, and
postmarked no later than March 31, 2015.
·
Enroll as a full
time undergraduate student in any accredited postsecondary institution within
the state of Florida for the 2015-2016 year.
·
Proof of
registration and social security number are required upon award of this
scholarship.
