Brooks Rehabilitation and the DCPS
District Athletic Office are providing a $5,000.00 college scholarship to two
seniors; one male and one female; planning to major in Sports Medicine and/or a
related discipline in college.
v Minimum unweighted GPA of 3.0
v Two Letters of Recommendation
v Must have served at least 75 service hours
v Must submit a copy of acceptance letter
v Must submit a 200-300 word essay on why you chose this
profession (Sports Medicine or related discipline) and tell how you want to
make a difference in your community or in the lives of others.
v Must be pursuing a degree in Sports Medicine or
related discipline
v Must be a 2015 graduate of a Duval County Public High School
v Must plan and be accepted to a two or
four year institution
